Stress-Induced Cardiomyopathy: A Need for Prospective Multicenter Trials
نویسنده
چکیده
Stress-induced cardiomyopathy (also called Takotsubo cardiomyopathy, apical ballooning, or broken heart syndrome) was first reported in 1991 as “myocardial stunning due to simultaneous multivessel coronary spasms: a review of 5 cases” by Japanese doctors, and is well known disease entity now. This disease primarily affects postmenopausal women after psychological or physical stress. Clinical characteristics are a triad of sudden onset of chest pain or dyspnea, electrocardiographic findings with ST segment elevation and evolutional T-wave changes, and a moderate elevation in cardiac enzymes mimicking acute myocardial infarction. As a precaution, coronary angiography is usually performed to differentiate from acute coronary syndrome. Even though many case and clinical study reports have been published, the mechanisms of stress-induced cardiomyopathy (SCM) are still unknown. The pathophysiology of SCM is includes vasospasm of coronary arteries; disturbance of microcirculation; obstruction of the left ventricular outflow obstruction; catecholamine-mediated myocardial stunning, which is an important link between emotional or physical stress and cardiac injury; hormonal interactions; and inflammation.Clinical Characteristics and Adverse Events

Transient left ventricular hypocontraction induced by emotional stress with immobilization: an animal model of tako-tsubo cardiomyopathy in humans?
To the Editor: I read with great interest the article by Ueyama et al in the Circulation Journal1 in which they demonstrated that emotional stress with immobilization in the rat induced reversible left ventricular apical ballooning that was normalized by pretreatment with adrenoceptor blockade. They had previously demonstrated the same stress-induced ST segment elevation, which returned to pre ...
متن کاملStress-induced cardiomyopathy complicating severe babesiosis.
A post-menopausal lady with severe babesiosis developed a basal-type stress-induced cardiomyopathy. Left ventricular function normalized at three months. We believe this is the first reported case of stress cardiomyopathy complicating severe babesiosis. Cardiac biomarker elevation disproportionate to the area of myocardial dysfunction, electrocardiographic changes, the patient's clinical condit...
متن کاملMulticenter randomized trial of cell therapy in cardiopathies – MiHeart Study
BACKGROUND Cardiovascular diseases are the major cause of death in the world. Current treatments have not been able to reverse this scenario, creating the need for the development of new therapies. Cell therapies have emerged as an alternative for cardiac diseases of distinct causes in experimental animal studies and more recently in clinical trials. METHOD/DESIGN We have designed clinical tr...
متن کاملIn Vitro Maturation of Oocytes in Women at Risk of Ovarian Hyperstimulation Syndrome-A Prospective Multicenter Cohort Study
Objective In vitro maturation (IVM) is an artificial reproductive technology in which immature oocytes are harvested from the ovaries and subsequently matured in vitro. IVM does not require ovarian hyperstimulation and thus the risk of ovarian hyperstimulation syndrome (OHSS) is avoided. In this multicenter prospective cohort study we assessed the live birth rate per initiated in vitro maturati...
متن کاملSurvival of Premature and Low Birth Weight Infants: A Multicenter, Prospective, Cohort Study in Iran
ABSTRACTBackground: The survival rate of preterm and low-birth-weight (LBW) infants depends on various factors such as birth weight, gestational age, and quality of care. The present study aimed at evaluating the survival rate of preterm and LBW infants, predictive factors, and the risk of mortality in three training hospitals of Mashhad and Tabriz cities. Methods: This prospective, cohort study...
